Hospice Caregivers Award Program

At Ballard-Durand Funeral & Cremation Services, every day we hear about the wonderful work hospice caregivers do for families in the Hudson valley:

To honor these dedicated people for the great care and comfort they bring to patients and their families, Ballard-Durand has created the Hospice Caregiver Award program.



We realize it takes special people to work in hospice and that there are wonderful caregivers throughout our area. This program is open to all Hudson Valley area hospices, whether they are for-profit or not-for-profit. Our goal is to honor great caregivers for the exceptional care they provide.

ELIGIBILITY


Any hospice employee or volunteer who provides care or support directly to a patient or their family is eligible for nomination including:

  • Nurses and nurse aides
  • Social workers
  • Bereavement staff and chaplains
  • Volunteers
  • Anyone who works directly with the patient and their family
  • Office support staff

HOW TO NOMINATE


You can nominate a hospice caregiver or co-worker who you believe exemplifies what it means to be a hospice caregiver. Please complete our online nomination form. Nominations may be submitted anytime throughout the year and individuals may be nominated more than once.

All nominations and information are confidential. Some restrictions apply. 

CAREGIVER AWARDS CEREMONY

Each month, one of the nominees submitted for that month will be selected to win a Hospice Caregiver of the Month Award. A Ballard-Durand team member will visit the winner’s hospice personally to deliver the award and take a photo of the award winner. Monthly winners will also be promoted on the Ballard-Durand Funeral & Cremation Services website and Facebook page.


In December 2017, Ballard-Durand will host a special Hospice Caregiver Awards Ceremony for all the nominees and all the Hospice Caregiver of the Month Award winners. The year’s nominees will each receive a small gift in appreciation for the wonderful care they provide. During the ceremony, one of these nominees will be awarded the Hospice Caregiver of the Year Award, a $1,000 scholarship, for their dedication and commitment to excellence. 


The $1,000 scholarship is intended to enhance the knowledge and skills of hospice care. The Hospice Caregiver of the Year Award winner may use their scholarship for a local continuing education program or conference, as well as for career-enhancing certifications. If the recipient is not able to take advantage of the scholarship, the scholarship will be provided to the hospice Executive Director to use at their discretion.


Nominees are eligible to win the Hospice Caregiver of the Year Award only once. However, nominees are eligible to win the Hospice Caregiver of the Month Award once every 6 months.
